關於 JUUL 電子煙集體訴訟
如果你在 2022 年 12 月 7 日之前購買過 JUUL 電子煙，就可以參與這宗集體訴訟，爭取總金額 $255 M 的賠償。原因是 JUUL 隱瞞了電子煙的成癮性，以及向未成年人非法出售電子煙。當然，JUUL 否認任何不當行為，只是同意提供賠償。
另外，如果你的購買金額低於 $300，通常不需要提供購買證明；超過 $300 則需要提供。
重要日期
最重要的日期是提交截止日： 2023 年 7 月 14 日。如果你有異議，也需要在這個日期前 opt out 或寫信給法院提出反對。

如果你曾收到這宗集體訴訟的 claim number，直接填寫即可。建議直接選擇網上填寫，沒有必要下載表格、花時間填好後再郵寄。
如果你沒有收到，但確實曾購買過 JUUL 產品，你可以選擇未收到 claim number，或者在以下連結直接提交新的 claim：
提交基本資料後，你需要填寫購買電子煙的時間。
以及購買電子煙的數量。
如果你申報的購買金額超過 $300，就需要提供購買憑證，例如收據、信用卡賬單、網上訂單的 confirmation page 等。
Documentation: At the end of this Claim Form, supporting documentation will be requested if you elect to submit your purchase information via documentation OR if the claimed quantity of purchases is greater than $300. Acceptable documentation includes receipts, bank/credit card statements, and/or pictures of online confirmation page (s) .“Self-prepared” documentation such as handwritten explanations are, by themselves, insufficient to receive payment.If you do not have your documentation available at this time, you should still submit the claim form. After your claim is reviewed, you will be given instructions by email as to how to upload your documentation. Without documentation your claim will be limited to $300.
